1. REFERENCES - If you have met with providers in the past, simply state their names, website or advertisement links, and contact information. At least two references are required.

2. IDENTITY VERIFICATION - Share something with me that shows your face and verifies your full name, such as a passport or driver’s license (you may block out your address!) Please attach a picture of the ID along with a sheet of paper that has today’s date on it - both the ID and paper must be in the same photo.
